Personal information for each person (applicant) over 18 years of age detailing:
- Complete schooling history
- Employment since 18th birthday
- Residences since 18th birthday.

- Full name
- Current address
- Date of birth
- Date of death and
- marital status
- Four passport-size pictures
- Photocopy of passport (information pages only)
- Birth certificate

| Separation agreement, child custody order (if you are under 16 years of age) |

| Proof of assets and property. (Bank statements for previous four months. Proof of Canadian bank account in your name if money has already been transferred to Canada) |

| Letter from school you wish to attend indicating acceptance. |
